• A statement that you have a good-faith belief the use is not authorized.
• A statement, under penalty of perjury, that the information is accurate and you are authorized to act.
• Your physical or electronic signature.

Counter-Notification

If you believe content was removed by mistake, you may submit a counter-notice to support@faithharborwebhosting.com including:

• Identification of the material and its prior location.
• A statement under penalty of perjury you have a good-faith belief removal was due to mistake or misidentification.
• Your name, address, phone, and consent to jurisdiction in your federal district (or where we are located if outside the U.S.).
• Your physical or electronic signature.

Repeat Infringers

We may terminate accounts of repeat infringers when appropriate.
